Further fixes to Elisp manual

* doc/lispref/frames.texi (Frame Layout): Index "tab bar" instead
of "internal tab bar".
(Implied Frame Resizing): Remove irritating hyphens.
* doc/lispref/windows.texi (Windows and Frames): Remove paragraph
relating 'minibuffer-window' to 'window-list'.
