(fill-paragraph): Consider a comment-start as

a paragraph starter when filling non-comments.
(fill-comment-paragraph): Use comment-search-forward.
Recognize paragraph starters and separators within comments.
Use adaptive-fill-prefix when it works.  Don't narrow while doing the fill.
