Fix structure of condition object in nested 'ert-fail'.

* lisp/emacs-lisp/ert.el (ert--should-signal-hook): Condition list
should be (SYMBOL . DATA), not (SYMBOL DATA).
* test/lisp/emacs-lisp/ert-tests.el (ert-test-fail-inside-should): Fix
unit test.
