74b19501bc Convert to PORTNAME/PORTVERSION.
Anomalies found:
 - autoconvert was missing "zh-" in front of PKGNAME
 - xcin (both 2.3 and 2.5) had invalid version strings
 - neither of xcin ports had NO_LATEST_LINK
 - telnet8-1.0 thinks PKGNAME is zh-telnet-1.0

Fixed all except the last (I wasn't sure if it was intentional).

Reordered vars where needed.  Added MAINTAINERS where needed, many
mkdir --> ${MKDIR}, install -> ${INSTALL_*}, etc.
