documentation for ports like qtcreator and qbs. Bump PORTREVISION
to reflect the update.

I've witnessed this port fail two separate times because it doesn't ensure
that the "build" directory has been created before it starts constructing
object files.
The patch moves the "doc" port in bsd.qt.mk from _USE_QT4_ONLY to
_USE_QT_ALL and bumps the PORTREVISION in devel/qt5 for the inclusion
of the qt5-doc port into the qt5 metaport.
Pre-Work for updating qtcreator to 4.0 for adding options to install
qt5-doc and qt5-examples to make qtcreator actually usable for
serious qt development.

plan to integrate this into the USES=tcl framework once 8.7.0 is released.

GDB's libiberty.h has a prototype of the basename() function it which
does not match the one that is part of POSIX, declared in libgen.h. This
is normally never visible, as GDB never includes libgen.h. On FreeBSD,
it unfortunately is, as our locally added copy of kgdb includes both.
Fix up libiberty.h to just include libgen.h. I'm currently discussing
with upstream how a clean fix should be done, but I guess that
requires more refactoring to the existing code. We'd better not bother
importing that and stick to this compact workaround.

- Version 3.0 is a major release that requires Ruby 2.0 compatibility
and removes deprecated functions.
- The columnar registry format introduced in 2.6 has been made the
primary format
- the registry data has been extracted from this library and put
into ruby-mime-types-data
- Additionally, mime-types is now licensed exclusively under the MIT
licence and there is a code of conduct in effect.

`__forceinline' with `inline __attribute__((always_inline))', which is
how upstream originally defines it in order to be understood by both
Clang and GCC, and don't force CHOSEN_COMPILER_TYPE as it is not needed
anymore. Bump port revision.
